BMI, waist circumference, blood glucose history, HOMA-IR estimate, family history, dietary pattern, activity level. Informed by ADA, ACC/AHA risk frameworks.
Hormonal Sub-Score
ADAM questionnaire (validated TRT screening), energy, libido, muscle maintenance, mood, sleep quality, last lab date. Correlates with established low-T diagnostic criteria.
Recovery Sub-Score
Pain location, duration, severity, mobility, sleep disruption, prior care, stress level. Aligned with Oswestry Disability Index and PROMIS physical function measures.
